Integrated psychosomatic service provision in a department of internal medicine compared with a traditional setting

In order to evaluate the integration of psychosomatic thinking in internal medicine, patient data and treatment schemes of two departments of internal medicine (Medizinische Klinik 2 (Head of department: Prof. Dr. H. Wedler) and Medizinische Klinik 3 (Head of department: Prof. Dr. W. Beischer)) of the Bürgerhospital Stuttgart (www.buergerhospital.de) with and without psychosomatic treatment are compared. Three main questions shall be answered by the study:

1. In which way is a hospital for internal medicine integrated in the psychosocial health care system of a big city like Stuttgart? (Evaluation of sociodemographic, diagnostic and psychosocial data of all patients within a period of 9 months admitted for inpatient treatment, treatments prior to admission, the way of admission, the treatment on the wards, the treatment after hospitalization and a catamnestic evaluation after 6 months).

2. Comparison of both hospitals for the patients admitted by emergency.

3. Cost-effectiveness-analysis of the additional psychodiagnostic and psychotherapeutic measures in the internal department working with integrated psychosomatic service provision.

Furthermore data about sociodemography, admission to the hospital, treatments before admission and work disability days and days in hospital are collected. At admission and discharge somatic and psychosomatic diagnoses according to ICD-10 are assessed and the Impairment Score (IS) (Beeinträchtigungs-Schwere-Score (BSS) nach Schepank) is rated. At admission and for the catamnestic evaluation, patients are given the SCL-90-R-questionnaire. During the study period from June 2000 to March 2001, 1806 patients were included.
